2010 Performance Report:
Open Heart Surgery.

Due to factors that include improved drug therapies, decreases in disease incidence and the economic downturn, there has been a national downward trend in open heart surgery procedures. Despite this trend, St.Vincent Heart Center of Indiana continues to have the highest-volume adult open heart surgery program in Indiana. Surgeons at St.Vincent Heart Center performed a total of 1,304 open heart procedures in 2009.

St.Vincent Heart Center provides a full range of open heart procedures, including:

• Coronary artery bypass surgery (CABG)
• Minimally invasive direct coronary artery bypass surgery (MIDCAB)
• Beating heart surgery off pump coronary bypass (OPCAB)
• Heart valve replacement and repair
• Transmyocardial revascularization (TMR)
• Robotic-assisted cardiothoracic procedures
• Ventricular assist devices (VADs)
• Cardiac transplantation
• Hybrid procedure: MIDCAB open heart surgeries with percutaneous coronary interventions in the same setting

The recent addition of a hybrid operating room has expanded open heart surgery capabilities at St.Vincent Heart Center of Indiana. This advanced operating suite allows interventional cardiologists to perform percutaneous coronary interventions in conjunction with surgeons performing open heart procedures.

St.Vincent Heart Center of Indiana performed 650 isolated CABG procedures in 2009. Our outcomes compare favorably with the STS registry, which calculates expected mortality rates for isolated CABG cases based on acuity. Mortality rates for isolated CABG cases at both our 86th Street and 106th Street & North Meridian campuses fall well below STS registry rates.

The STS registry recommends the use of internal mammary arteries for isolated CABG procedures at a rate of 94.7%. In 2009, the combined rate for IMA grafting of isolated CABG procedures for both campuses was above the STS rate at 97.4%.

Our figures for average length of stay following isolated CABG surgery demonstrate that we provide high-quality care in a cost-effective manner. These figures are below STS standard, and we improved in 2009 from 8.2 days to 7.8 days.

While patient volume for valve procedures decreased slightly in 2009, St.Vincent Heart Center remains the largest program in Indiana treating valvular diseases of the heart. In 2009, 457 patients were treated surgically for valvular disease.

Valve surgeries continue to be a significant percentage of open heart surgeries at St.Vincent Heart Center. In 2009, valve surgeries accounted for nearly one-third of open heart surgeries.

St.Vincent Heart Center provides advanced treatment for valvular diseases. Treatments include minimally invasive mitral valve repair, complex aortic valve repair and various types of valve replacements. At 3.4%, mortality rates for both aortic and mitral valve repair at St.Vincent Heart Center are well below the STS average for 2009. Mortality rates of 2.7% for CABG/Valve replacement also fell below the STS average.
